Minor rules clarification

While this is a minor rules clarification, it is actually a pretty helpful update. When play testing a question was asked about what happens when a character takes a wound and all wound slots are filled. The rules as previously written were pretty ambiguous. So, I clarified the rules to better explain what happens to existing wounds when the character gets a new wound.

There were some minor formatting updates as well.
